Training Description This presentation will focus on the science and experience of building inter-professional health care teams to investigate questions that benefit community caregivers caring for frail older adults with chronic illness and dementia. Using inter-professional models of care, Dr. Pastor will illustrate her professional experiences bringing caregivers together to meet the needs of caregiving families. Policy implications will be addressed.
