Coming Together: Building Inter-professional Research and Practice

Training Description
This presentation will focus on the science and experience of building inter-professional health care teams to investigate questions that benefit community caregivers caring for frail older adults with chronic illness and dementia. Using inter-professional models of care, Dr. Pastor will illustrate her professional experiences bringing caregivers together to meet the needs of caregiving families. Policy implications will be addressed.

Learning Objectives
By the end of the training, participants will be able to:

  • State two models of inter-professional health care team management for a patient with chronic illness.
  • Explain how family caregivers for persons with dementia can be supported in community care settings.
  • State two policy implications of managing community-dwelling older adults with dementia.
